Standard Operating Procedures

For Elementary, High School and TVET
Current School Year

  • AUTHENTICATION OF DOCUMENTS (Client)
      1. Fill out the request form at the Registrar’s Office.
      2. Present the original copy of document/s to be authenticated.
      3. Provide a photocopy of the said document/s.
      4. Pay the total amount at the Accounting office.
      5. Present the Original Receipt to the Registrar’s Office.
      6. Releasing of document/s would be within the day.
      7. Requester signs the Log Sheet after receiving the document/s.
  • ISSUANCE OF CERTIFICATIONS
      1. Fill out the request form at the Registrar’s Office (Indicate the purpose of request)
      2. Pay the total amount at the Accounting Office.
      3. Present the OR to the Registrar’s Office.
      4. Get the claim stub and return on the specified date of release.
      5. Releasing of document/s would be within the day or subject to the kind of certificate needed (utmost is 1 day).
      6. Requester signs the Log Sheet after receiving the document/s.
    Certificates (issued within the day of request)
      - Certificate of Enrollment
      - Certificate on the use of English as medium of instruction
      - Certificate of Completion
      - Certificate of Candidacy for graduation
      - Certificate on Transfer Credentials/Honorable Dismissal
      * Other certificates which require new content would be released a school day after.
  • DIPLOMA ( Alumni – Request for a 2nd copy )
      1. PRESENT AN AFFIDAVIT which states the reason for a second copy.
      2. Fill out the request form.
      3. Pay the total amount at the Accounting Office.
      4. Present the Original Receipt to the Registrar’s Office.
      5. Get the claim stub and return on the date of release.
      6. Present the claim stub to get the requested document.
      7. Requester signs the Log Sheet after receiving the document.
    (Records Assistant)
      - Requests the client for an official ID.
      - Verifies the student’s name in the database.
      - The diploma can be released earlier if the number of requested document is not voluminous. Availability of the signatories should also be considered.
  • N.B. Acceptable REASONS for requesting second copy of diploma
      Lost due to natural disasters like earthquake, flood, typhoon Torn, damaged, burnt by fire
      - The requester must present a duly notarized Affidavit of Loss
      - The affidavit must contain the following:
      1. Primary information of the alumnus/alumna.
      2. Reason why the document/s was/were lost.
      3. Documents to be requested.
      4. Data about the Notary Public under the Notarial Commission
  • DIPLOMA (for Authorized Representative)
      1. Present the following to the Registrar’s Office:
      1. Duly notarized authorization letter
      2. Photocopy of student or alumni valid ID.
      3. Photocopy of authorized representative valid ID.
      2. Fill out the request form.
      3. Pay the total amount at the Accounting Office.
      4. Present the Original Receipt to the Registrar’s Office.
      5. Get the claim stub and return on the date of release.
      6. Present the claim stub to get the requested document.
      7. Requester signs the Log Sheet after receiving/claiming the document.

III. TRANSCRIPT OF RECORDS (Form – 137)

  • Personal Request
      1. Fill up the request form (Indicate the purpose of the said request)
      2. Pay the total amount at the Accounting Office
      3. Present the Original Receipt to the Registrar’s Office.
      4. Get the claim stub and return on the date of release.
      5. Present the claim stub to get the requested document (4 working days)
      6. Requester signs the Log Sheet after receiving/claiming the document.
  • With Authorized Representative
      1. Present the following documents to the Registrar’s Office:
      1. Authorization Letter signed by the owner of the requested document.
      2. Photocopy of student or alumni valid ID.
      3. Photocopy of authorized representative valid ID.
      2. Fill up the request form. Accounting Office.
      3. Pay the total amount at the Accounting Office.
      4. Present the Original Receipt to the Registrar’s Office.
      5. Get the claim stub and return on the date of release.
      6. Present the claim stub to get the requested document (3 working days)
      7. Requester signs the Log Sheet after receiving/claiming the document
  • REPORT CARD
      1. Present any of the following documents to the Registrar’s Office:
      1. Approved letter from the Principal duly notarized
      2. Affidavit of Loss
      2. Fill up the request form.
      3. Pay the total amount at the Accounting Office.
      4. Present the Original Receipt to the Registrar’s Office.
      5. Get the claim stub and return on the date of release.
      6. Clerk will provide printed copy of the Report Card using an ordinary paper.
      7. Present the claim stub to get the requested document (4 working days)
      8. Requester signs the Log Sheet after receiving/claiming the document.

TVET
Procedures for Requesting Document

I. TRANSCRIPT OF RECORDS

  • Personal Request
      1. Proceed to TVET Clerk for the verification of Financial Status.
      2. Proceed to the Accounting Office to settle accountabilities (if there’s any)
      3. Present the Official Receipt to TVET clerk for signature of Request for Students Records (RSR) form.
      4. Proceed to the Technical Director’s Office for approval of the request (look for the Secretary of the Technical Director).
      5. Submit the form to the Registrar’s Office together with the Official Receipt.
      6. Fill up the additional information request stamped at the back of the form.
      7. Get the claim stub and return on the specified date.
      8. Requester signs the Log Sheet after receiving/claiming the document.
  • With Authorized Representative
      1. Present the following documents to the Registrar’s Office:
      1. Authorization Letter duly notarized
      2. Photocopy of student or alumni valid ID
      3. Photocopy of authorized representative valid ID.
      2. Follow the process listed above. (STEPS 1-8)
      3. Requester signs the Log Sheet after receiving/claiming the document.

TVET
Steps to Apply For a Special Order From TESDA

(Registrar’s Office)

  • Requester
      1. Prepare the following requirements. (All original copy)
      1. High School Diploma
      2. Transcript of Records (TOR) issued by the Registrar’s Office
      3. TVET Diploma
      4. Certificate of Training issued by TVET
      5. OJT Certificate
      6. NC / RWAC (for WTR programs)
      7. Marriage certificate (if applicable)
      2. Provide a photocopy of the documents mentioned above.
      3. Submit to the Registrar’s Office the original and the photocopy of the documents for authentication / verification.
    NOTE: The TVET Records Assistant submits all documents to TESDA. TESDA releases the Special Order after five (5) working days from submission of documents.

Steps to Apply for NEW SCHOOL ID

Lost ID:
  • Requester
      1. Present an Affidavit of Loss to the Registrar’s Office.
      2. Fill up the request form. (Please see downloadable forms) Have the form signed by the Principal. Once accomplished, submit the form to the Registrar’s Office.
      3. Pay the total amount at the Accounting Office.
      4. Present the Original Receipt to the Registrar’s Office.
      5. Claim the new school ID at the Registrar’s Office. It will be released a school day after the request and payment have been made.
